Required School Physicals; May 14 Appointments
The Pennsylvania School Health Act requires a medical examination for students entering kindergarten or first grade, sixth grade, and eleventh grade.
Families have two options for completing this requirement:
Option 1: Visit Your Family Physician (Recommended)
We encourage families to schedule the exam with their own physician, who has a more complete understanding of your child's medical history. Completed physical forms should be submitted to your school nurse.
Option 2: Schedule a School Physical
If preferred, your child may be examined by our school physician, Dr. Louis Gianguilio, at no cost. School physicals will take place:
- Wednesday, May 14
- Appointments (required) begin at 2 p.m.
- Northley Middle School (2801 Concord Rd, Aston)
To Schedule a School Physical:
- Call Mrs. Betsy King, RN at 610-497-6300 x6505 by Monday, May 12 to schedule an appointment. Please note: No students will be examined without a scheduled appointment.
- Download the physical form on the Penn-Delco Health Services page or request one from your school nurse.
- Complete the Parent History section of the form before the appointment.
- Accompany your child to Northley at the assigned time with the completed form.
Please note: Immunizations will not be given at this appointment.
